Immunizations
Utah Immunizations
Utah requires proof of the following immunizations or an approved exemption for all students attending Kindergarten through 6th Grade:
- 5 doses of DTaP/DT (Diphtheria, Tetanus, Pertussis)
- 4 doses of Polio
- 2 doses of MMR (Measles, Mumps, Rubella)
- 3 doses of Hepatitis B
- 2 doses of Hepatitis A
- 2 doses of Varicella (Chickenpox)
Exemptions
Utah Code Section 53G-9-303 outlines the rules for exempting a student from school-required vaccines.
Types of Vaccine Exemptions
- Medical Exemption: Your child can skip a vaccine if a doctor or licensed healthcare provider signs a note stating that getting the shot would harm your child's health or endanger their life. You must turn in this doctor's note along with an official exemption form to the school.
- Personal or Religious Exemption: Your child can skip a vaccine if shots go against your personal or religious beliefs. You must fill out and submit an official exemption form to the school stating this belief.
Key Rules Parents Should Know
- Good Until Graduation: Once you turn in an exemption form, it stays valid for as long as your child is in the public school system. You do not need to renew it every year, and it follows your child if they change schools.
- In-Person Classes Allowed: If a school offers both online and in-person classes, they cannot force an exempt student to do online learning just because of their vaccine status.
- Health Outbreaks: The local health department still has the authority to step in and take safety measures if there is an outbreak of a contagious disease.
How to Exempt Your Child or Children from Immunization
You must first take an online education module. The exemption you get when you complete this module is only for students in PK-12. You must provide a copy of the completed form to the school. For a medical exemption from the required immunizations, the student’s legally responsible adult must give the school the completed form AND a written statement signed by a licensed healthcare provider stating in part, “due to the physical condition of the student/child, administration of the vaccine would endanger the child’s life or health.”
